• On the Pay line, write Self or your full name exactly as it appears in bank records.
• Write the amount in words on the designated line, ending with the word Only.
• Enter the withdrawal amount in figures in the box provided, followed by a slash and hyphen (/-).
• Sign clearly at the bottom-right corner, matching your registered bank signature.
