To change your surname on a PAN card, you must submit a 'Request for New PAN Card or/and Changes or Correction in PAN Data' application. This process can be completed online via the official Protean PAN website (formerly NSDL e-Gov) or the UTIITSL portal. Select the 'Changes or Correction in existing PAN Data' option and fill out the form. You must tick the corresponding box next to the Last Name/Surname field. Required supporting documents, such as a marriage certificate or an official Gazette notification for name change, along with identity and address proof, must be submitted with the application.
